We request all families using Hillingdon's children’s centre services to complete a registration form to ensure we have correct contact details for you and your family. The information you provide will assist in providing activities that best meet your family needs and support in the planning of services in the future.

By completing this registration form, you will be registered at all children’s centres across Hillingdon. In addition to this form, we will request that you complete an annual update form or an additional update form if your circumstances change.
